Arashi Mikami

Arashi Mikami (三神 嵐, Mikami Arashi) is the main male protagonist of the anime and manga series Triage X. He is a handsome yet socially awkward high school student who secretly works as an operative for Black Label, a vigilante organization that targets dangerous criminals beyond the reach of law enforcement.

As a child, Arashi survived a devastating terrorist attack that claimed the life of his best friend, Ryu Mochizuki. Following the incident, Arashi received multiple organ transplants from Ryu, a life-saving procedure that physically tied him to his fallen friend. On missions, he conceals his identity by wearing a full-face, bulletproof motorcycle helmet, allowing him to operate in anonymity while carrying out Black Label’s high-risk assassinations and covert interventions.

A distinctive feature of his hairstyle is that some of his frontal bangs are longer than the others, slightly framing his face. Arashi Mikami is a deeply introspective and emotionally restrained individual, shaped by the trauma of his past and the demands of his role in Black Label. During missions, he exhibits an intense focus often described as tunnel vision directing all of his attention toward neutralizing targets labeled “Black.” In quieter moments, he frequently engages in internal monologues, reflecting on his actions and the moral implications of both his choices and the world around him. He is particularly haunted by the memory of his deceased friend Ryu Mochizuki, whose voice continues to guide his thoughts.

Socially, Arashi is considered awkward and emotionally distant. He frequently displays behavior that sets him apart from his peers, such as randomly standing up during class, falling asleep unexpectedly, or failing to respond appropriately to social cues. His disinterest in typical teenage behavior most notably his lack of enthusiasm when invited to join his male classmates’ so-called “Big Titty Club” often leaves those around him surprised or confused.

Despite his stoic behavior, Arashi is not emotionally numb. History

Little is known about Arashi Mikami’s early life aside from the traumatic event that reshaped his future. As a child, he was close friends with Ryu Mochizuki, the son of the esteemed surgeon Dr. Masamune Mochizuki. The two boys shared an ordinary childhood, often playing games together and dreaming of becoming heroes.

Their lives changed forever during a terrorist attack at the International Medical Conference in Tobioka City, an incident that resulted in massive casualties. Arashi and Ryu were the only survivors. During the explosion, Arashi shielded Ryu with his own body, sustaining catastrophic injuries in the process. Although Arashi’s body was critically damaged, Ryu’s body remained intact, though he suffered a severe head trauma that left him brain dead.

Upon arriving at the scene, Dr. Mochizuki made a harrowing decision. Declaring his own son beyond saving, he labeled Ryu with a Black Label and used his still-viable organs to save Arashi’s life through a groundbreaking surgical operation. The transplant allowed Arashi to survive, but the experience left him emotionally scarred and physically altered.

After recovering, Arashi found himself struggling with the weight of his survival, the loss of his friend, and the responsibility of the second chance he had been given. This emotional burden instilled in him a strong sense of purpose and duty. No longer willing to be ruled by fear or indecision, he resolved to live life with clarity and conviction, an ethos that guides his actions as a member of Ampule One, the special ops unit of Black Label. His unwavering loyalty to Dr. Mochizuki and his teammates stems from this pivotal moment in his past.
